2016 SAFE EMPLOYEE AWARD
This award is designated for an individual that affects safety at the jobsite level (e.g., senior supervisor, superintendent, foreman, craftsperson).
Please complete this application in its entirety. Failure to follow the application directions may result in rejection of your submittal. This form must be completed by Friday, January 15, 2016.
To be eligible for this award, the nominee’s employer must be an ACI member. Additionally, if the nominee’s employer has had a willful violation or occupational-related fatality during 2015, the nominee may not be eligible for an award.
If the nominee’s employer receives a willful violation or has a fatality after submitting this application, ACI must be notified. The nominee may not be eligible for an award, and if they are a finalist, they may be removed from the eligible candidate list.

Why does this employee stand out from the safety perspective? Explain the thing(s) that you feel this employee does better than everyone else. Examples could include:
- Unique safety initiatives this employee may have created,
- How they exceed safety expectations of their employer,
- How they influence other employees,
- How they have overcome obstacles, and/or
- How they demonstrate safety leadership skills.
The information in each applicant’s narrative will be used to determine the winners of ACI’s 2016 Safe Employee Award. Provide enough information to fully explain why this employee is deserving of this award.

- Minimum of two and maximum of four electronic photos of this individual,
- High resolution company logo, and
- Copy of the most recent OSHA 300 Log.
If nominating an employee outside of your company, please provide contact information to ACI for the appropriate person to obtain this information from.
